Job Description:

Plan, direct and administer nursing services as a part of the interdisciplinary team, while under the supervision of the Nurse Manager or RN Shift Supervisor.
  1. Encourages and models team approach to accomplish the Luther Manor mission of providing excellent care and services to customers (assists others as needed).
  2. Follows State and Federal regulations, as outlined in the Employee Handbook, Nursing Department Policy and Procedure Manuals, Human Resources Manual and Resident Bill of Rights, and adheres to safety rules.
  3. Performs duties within the RN scope of practice as determined by Wisconsin Board of Nursing.
  4. Reports potential/actual safety hazards to Nurse Manager, Nursing Supervisor and/or Security immediately.  When able, will implement corrective actions.
  5. Assesses, plans, implements, and evaluates the nursing care given to residents, documenting findings per Luther Manor policy and procedure (i.e. Progress Notes, General/Admission Observations, Risk Assessments, 24 Hour Change of Condition Report Sheet, etc.)
  6. Promotes and protects the rights of residents by maintaining confidentiality, dignity and privacy.
  7. Administers medications and provides ordered treatments to residents.
  8. Monitors drug control and supplies (shift to shift count, expiration dates).
  9. Makes frequent rounds on residents to observe, examine, and evaluate resident care, inspect environment, and evaluate staff interactions.
  10. Assesses, documents and monitors residents for changes in physical and emotional condition (subtle and acute); notifies Nurse Manager/RN Shift Supervisor, physician and responsible parties promptly.
  11. Receives and transcribes physician orders.
  12. Provides resident/family education as needed (i.e. medication teaching, diabetic teaching, etc.)
  13. Fosters and promotes effective relationships with residents, families, and visitors.
  14. Promptly reports actual or suspected abuse, neglect, misappropriation and injuries of unknown origin to the Nurse Manager or RN Shift Supervisor; reports any family or resident complaints, resident change of condition, injury or incident to Nurse Manager or RN shift supervisor.  Completes documentation of same per Luther Manor policy and procedure.
  15. Updates the Care plan and CNA Care Assignment Sheets as needed.
  16. Communicates with personnel in other departments/shifts to facilitate appropriate resident care expectations.
  17. Assigns CNA’s work via the Group Assignment Sheets on the unit.
  18. Supervises and directs the care provided by the LPNs and CNAs on the assigned unit.
  19. Communicates expectations to LPNs and CNAs and ensures that they have the resources, tools and information to accomplish the delivery of care and services to each resident.
  1. Assists with orientation of nursing staff (RNs, LPNs, CNAs, Unit Secretaries).
  2. Provides input into evaluations of CNAs.
  3. Other duties as assigned.

Job Requirements:

  1. Current Wisconsin licensed Registered Nurse.
  2. Work with the realization that errors may have serious consequences for residents and coworkers.
3.     The ability to speak to and hear residents and staff; also to read and write English.
  1. The ability to deal tactfully and effectively with residents and staff.
  2. The mobility to spend an average of 70% of the work shift standing and/or moving about the work area.
  3. The physical strength to move or assist residents as needed following their plan of care.
  4. Must be able to push, pull or lift 50 pounds.
8.     Maintains knowledge of current nursing practice and regulations through attendance at seminars, inservices, etc.
