About Guild Incorporated:
Guild Incorporated helps individuals with mental illness lead quality lives. We provide an array of recovery-focused, community treatment and rehabilitation services for individuals 16 and older who have serious mental illnesses such as schizophrenia, schizoaffective disorder, and bipolar disorder.  On any given day, Guild staff serves nearly 900 individuals in the Twin Cities metro area.  Services are mobile, provided in the community when and where needed.
This position is responsible for providing nursing services and oversight for individuals who have serious and persistent mental illness.  Perform recipient assessments upon admission to all programs. Assists with the admission process and determines immediate needs.  It provides a full range of community health nursing, health management, and community health services to individuals with serious and persistent mental illness. Provide on-call nursing one week per month on an as needed basis.

Key Responsibilities:
  • Provide crisis assessment and intervention so recipients receive the assistance they may require to be safe.
  • Provide flexible individualized support so recipients can remain safe in the program and can lead self-directed lives.
  • Assess recipients' medical needs to identify goals of service.
  • Promote an inclusive, diverse, culturally competent and respectful work environment.


Job Requirements:

Desired Education and Experience:

  • Must be a graduate of an accredited nursing program and possess a valid LPN or RN certification to practice in Minnesota.
  • Must meet the staff qualification requirements of the Minnesota Department of Human Service Rules for Intensive Residential Treatment Programs.
  • Minimum one year of experience working with people who have severe and persistent mental illness required.
  • Experience or knowledge of chemical dependency issues for adults with mental illness preferred.

Other Requirements:

  • Must be ambulatory and able to negotiate stairways. 
  • A valid MN Class ‘D’ Driver’s License, reliable vehicle, satisfactory driving record and necessary insurance.
  • Must be able to lift and carry heavy weights that could be up to or even exceed 40 pounds.
Must not be actively receiving services from Guild Incorporated and must not have received services within the last two years
