About Guild Incorporated: Guild Incorporated helps individuals with mental illness lead quality lives. We provide an array of recovery-focused, community treatment and rehabilitation services for individuals 16 and older who have serious mental illnesses such as schizophrenia, schizoaffective disorder, and bipolar disorder. On any given day, Guild staff serves nearly 900 individuals in the Twin Cities metro area. Services are mobile, provided in the community when and where needed. Maureen’s House provides 24/7 crisis stabilization services in a residential setting to help recipients experiencing a psychiatric crisis to stabilize without becoming homeless and, whenever possible, without hospitalization. Guild South provides 24/7 intensive residential treatment services in a safe, home-like environment for recipients with serious and persistent mental illness to develop and/or enhance their mental health stability. 

Purpose: This position is responsible for providing on-call crisis nursing for individuals who have serious mental illness. Perform recipient wellness assessments upon admission to all programs. Assists with the admission process and determines immediate needs. It provides a full range of community health nursing, health management, and community health services to individuals with serious and persistent mental illness. Provide on-call nursing one week per month on an as needed basis.

Key Responsibilities:
• Provide crisis assessment and intervention so recipients receive the assistance they may require to be safe.
• Provide flexible individualized support so recipients can remain safe in the program and can lead self-directed lives.
• Provide specialized nursing services to residents; communicate needs of recipients and share nursing experiences.
• Educate recipients to independently, and safely, manage their medications in preparation for living in the community.
• Provide wellness assessment and intervention so recipients receive the assistance they may require to be safe.
• Assess recipients’ medical and support needs and identify readiness for change, set goals for interventions accordingly.
• Promote an inclusive, diverse, culturally competent and respectful work environment.

Job Requirements:

Desired Education and Experience: The Licensed RN in the state of Minnesota shall be qualified in at least one of the following ways:
• Holds a Bachelor's Degree in nursing and has had two years of previous experience with people with serious and persistent mental illnesses;
• Holds a three-year diploma in nursing and has had a minimum of two years of previous experience with people with serious and persistent mental illnesses.
• Experience in public health or other community-based nursing is desired. Other Requirements:
• Must be ambulatory and able to negotiate stairways.
• A valid Driver’s License, reliable vehicle, satisfactory driving record and necessary insurance.
• Must be able to lift and carry heavy weights that could be up to or even exceed 40 pounds.
• Must not be actively receiving services from Guild Incorporated and must not have received services within the last two years
